I specifically want format to not be optional since we already do have some nominations that were physical only
Aside from that the optional extra general notes is a good idea! I’ll add that

I’ve seen people leave it out. And I personally just assume it’s “physical/digital” and probably didn’t always (or ever) check if there is a physical version when I nominate.
You could also put something in the template like “please note restrictions on availability such as lack of a digital Edition”.
