Add a Kindle support checkbox to book request form

Description of your request or bug report: It’d be really handy if there was a (pre-selected) checkbox on the book request form where, if you unselected it, the generated pages for the book/series didn’t have Kindle automatically shown as an option. I’ve submitted quite a few older books with no digital version, and I forget 90% of the time to mention it on the book form so I end up having to go change it manually.

Would be nice if there was an option to see “Audio Book” or “Audible” available as well,

Approved. This may be relegated to obsolete, as I think I’ll be changing the whole system of submission in the next few months to be more automated, we’ll see. Regardless, good idea.
